Primary care and recovery, together
Your health is connected, and your care should be too. When you see both our primary care and recovery providers, they can share your plan (with your permission), so medications, lab work and follow up all line up. You don’t need to be in recovery to be a primary care patient with us.
Becoming a patient
Call us at (517) 225-4483 to book your first visit. To save time, complete the Primary Care New Patient Packet before you come in and bring your insurance card and photo ID. If you have questions about your insurance, our team is happy to check your coverage when you call.
